Resumo

Introdução: a fisiopatologia e progressão da doença pulmonar na fibrose cística (FC) determinam a necessidade de intervenções fisioterapêuticas. Dentre elas, a manobra de compressão com descompressão brusca (CDB) pode ser utilizada, sendo raras as evidências quanto a sua indicação. O efeito de sua aplicação na mobilidade diafragmática (MD) ainda é desconhecido. Objetivo: avaliar o efeito da CDB na MD de crianças com FC. Métodos: estudo descritivo incluiu crianças de 1 a 14 anos com diagnóstico confirmado de FC. Aplicou-se os escores clínicos Cystic Fibrosis Clinical Score e Cystic Fibrosis Foundation Score para garantir estabilidade clínica no momento da intervenção. Após o registro de dados de colonização bacteriana, genótipo e gravidade da doença pelo Escore de Schwachman-Doershuk, obtidos em prontuário, realizou-se avaliação antropométrica. Realizou-se 3 registros ultrassonográficos (NanoMax) da excursão diafragmática em respiração espontânea (T1 ), em decúbito dorsal elevado a 30°. Na sequência, aplicou-se 10 manobras de CDB em ápices e em bases pulmonares, bilateralmente, e registrou-se novamente 3 medidas (T2). Considerou-se o maior valor das 3, tendo estes variação máxima de 10% entre os outros valores menores. A análise estatística foi processada no software IBM SPSS® 20.0 e empregou-se análise descritiva e de frequências. Resultados: participaram 11 crianças (63,63% meninas), com média de idade de 8,38±4,64 anos. A excursão diafragmática basal apresentou uma média de 1,72±0,58 cm e, no T2 , aumentou numericamente para 1,90±0,48cm. Conclusão: a maioria dos casos deste estudo demonstrou aumento numérico na MD no T2.

Abstract

Background: cystic fibrosis (CF) pathophysiology and progression of lung disease determine the needs for physical therapy interventions. Among them manual chest compression and decompression maneuver (MCCD) can be used, even though it has scarce evidence. The effects of MCCD in diaphragm mobility (DM) is unknown. Aim: evaluate the effects of MCCD on the DM in children with CF. Methods: Descriptive study including children from 1 to 14 years old with CF diagnosis confirmed. Applied Cystic Fibrosis Clinical Score and Cystic Fibrosis Foundation Score to ensure clinical stability at the time of intervention. After recording data on bacterial colonization, genotype and disease severity with SchwachmanDoershuck score (SDS), collected from medical records, anthropometric evaluation was made. Three ultrasound (US) quantifications (NanoMax) of the DM in spontaneous breathing were perfomed (T1), in supine position elevated to 30°. Then, applied 10 MCCD on apex and bases, bilaterally. Another US measurements were made to collect the 3 final records (T2). The highest value of each participant was considered for analysis, with it not varying 10% among the others lowest values. It was processed by IBM SPSS® 20.0 software and employed a descriptive and frequency analysis. Results: 11 C/E (63,63% girls) participated, with an average age of 8,38±4,64 years. The average basal DM was 1,72±0,58cm and, in T2 , increase to 1,90±0,48cm. Conclusion: most cases on this study showed numerical increase on their DE.
